NDIS Assessments

Formal assessments provide evidence to the NDIS of the supports you need to achieve your goals.

Functional Capacity Assessment

A functional capacity assessment looks at your functional performance in your activities of daily living, both in the home and out in the community. This can include assessment of:

🛀 How you manage activities of daily living at home

🏠 Your home environment and any need for home modifications

🚌 Transport requirements

🧑🏼‍🤝‍🧑🏽 Your social supports

A functional capacity assessment can help to establish clarity around your goals and articulate the specific supports you need to participate in life the way you want to.

Supported Independent Living

Supported Independent Living (SIL) includes any funded service supports that enable you to live more independently. SIL assessments are very comprehensive, and include assessment of your functional status and the amount and types of supports that you need to maintain and improve the quality of your life.

During a SIL assessment, our therapists take a comprehensive look at, among other aspects of your life:

🏠 Your home environment and any need for modifications

🍳 Your support requirements for activities of daily living

🛒 Community access requirements

 

Specialist Disability Accommodation

Specialist Disability Accommodation (SDA) is accommodation for NDIS participants who require specialist housing that caters for their very high support needs. We provide a comprehensive assessment of your current and future needs to determine whether SDA is a reasonable and necessary support for you, and make a recommendation to the NDIS about your eligibility.
